<p>Summary: <p>In the interest of extending supports and services to students and families in the elementary schools in a Family of Schools, it is proposed that a Youth Liaison Worker position be created to work directly with elementary school teams to offer families direct support and community connections to the Youth Hub and community based programming, as per need.Duties and Responsibilities: To work with feeder school teams and their Secondary School to facilitate effective transitions between feeder schools and Secondary Schools to assist with building networks of support for potentially in risk students.<p> Build capacity with families through community connections, assisting with access to community based resources and programs, <p> Foster and build positive relationships between schools and families.<p> Seek out and build relationships as well as facilitate connections between community groups / programs that could benefit schools or families <p> Provide direct support to youth and families in accessing the Youth Hub at their Secondary School and community resources<p> Promote and facilitate connections with Youth Hub at their Secondary School<p> Provide direct in school support to students in need at the elementary feeder schools and work with the school teams to develop a plan of support for the youth and their family Qualifications: Secondary School Diploma (Grade 12)<p> Knowledge of Durham Region and Resources within the Region<p> Ability to connect with youth and their families to build positive relationships<p> Ability to travel between involved schools<p> Able to flexibly and creatively manage their own work schedule<p> Proven ability to work with in risk youth and form plans that will support them. <p>This is a 35 hour per week, permanent position.
