Our medical clinic is currently seeking a dedicated NP to join our team as a member of the Nurse Practitioner Primary Care Program. As a Nurse Practitioner at our clinic, you will play a crucial role in providing high-quality patient care and promoting wellness within our community. *\*\*Erin Ridge Family Health Centre\*\** · We will assist our Nurse Practitioner in signing up for the Nurse Practitioner Primary Care Program and will assist the NP in fulfilling the ongoing program requirements per the program guide. · Supportive and inclusive work environment · Work-life balance with flexible scheduling options · Brand new facility · Located in the rapidly growing neighborhood of St. Albert, Erin Ridge · Uncomplicated patient population · Highly trained admin & support staff, with low turnover · Continentally educated physician team, all welcoming to train new graduates and newly licensed physicians, including International Medical Graduates · Fully computerized clinic with Accuro EMR; Easy to learn and customizable to individual user preference · Attached with next door neighbor Erin Ridge Pharmacy · Neighbors with Insight Medical Imaging, with in-house radiologists · Less than 7 minutes from Sturgeon Hospital & Alberta Precision Labs · Free parking We currently have a team of: · 6 General Practitioners · Psychologist Specialists: · Internal Medicine · Psychiatrists (x2, virtual consults) · Gynaecologist Current Admin Staff: · 4 MOA?s · 1 LPN · 1 RN · Assistant Clinic Manager · Clinic Manager · 2 In-house Billing and Reconciliation clerks *\*\*Key Responsibilities:\*\** - Conduct comprehensive patient assessments, including medical history review, physical examinations, and diagnostic testing. - Develop and implement individualized care plans in collaboration with patients, families, and other healthcare professionals. - Diagnose and treat common acute illnesses, injuries, and chronic conditions, including but not limited to hypertension, diabetes, and respiratory infections. - Prescribe medications and perform minor procedures within the scope of practice. - Educate patients on preventive care, health maintenance, and treatment options to empower them to make informed decisions about their health. - Maintain accurate and up-to-date medical records, including patient charts, progress notes, and treatment plans. - Collaborate with physicians, nurses, and other interdisciplinary team members to ensure continuity of care and optimal patient outcomes. - Participate in quality improvement initiatives and adhere to regulatory standards and best practices in healthcare delivery. - Maintain a minimum patient panel of 900 patients. Prior to having a patient panel of 900 patients, NP must provide walk-in availability - Utilize CII/CPAR to manage patient panels - Share patient panel with HQCA for quality improvement (Panel reports) - Provide Alberta Health with yearly annual audited financial reports that are in accordance with Canadian Accounting Standards for not-for-profit organizations. - Report quarterly on key performance indicators that measure the progress and quality of primary care delivery in their settings ? performance measures and evaluative processes will be developed by Alberta Health. - Hold adequate personal liability protection with CNPS Link to requirements in detail: https://open.alberta.ca/dataset/00a02c21-141b-46be-af2c-528ef6ee29a6/resource/1fc45515-397e-4984-9b23-5f1ca6c76754/download/hlth-nurse-practitioner-primary-care-program-guide-2024-04.pdf *\*\*Qualifications:\*\** - Current licensure as a Nurse Practitioner valid in- LocationSt. Albert, AB
