*Job Title:* Case Manager
*Position Type: *Full-Time (40 hours weekly)
Caritas School of Life strives to support individuals facing the challenges of addiction and mental health concurrent disorders through healing provided in a day and residential therapeutic community program. The role of a Case
Manager is to work with clients to achieve safe, realistic and reasonable goals by supporting all individuals in fostering the tools to take on the challenges associated with addiction and mental health concurrent disorders.
Additionally the Case Manager is expected to collaborate with the interagency team on a daily basis to help support the caseload. The purpose of the position is to help clients while in the bed based and out patient programs.
*Duties and Responsibilities:*
* Inquiry, assessment to include Gain Short Screener, intake, and discharge process for the caseload
* Conduct a full screener for all inquiries to determine suitability.
* Accountable for reports to include monthly data management, end of shift case notes where applicable
* Maintenance of all client related forms
* Management of all relevant agency information systems (eg. Connex/Datis/Catalyst reporting)
* Acting as a mentor and a role model by following core principles, demonstrating honesty, integrity, and willingness to listen to client issues.
* Report all incidents (no matter how minor)
* Ensure legislation, agency policies, standards and guidelines are being followed.
* Demonstrate strong teamwork and leadership skills within the interagency team.
* Perform other duties as required.
* Manage crisis situations and emergencies by taking appropriate steps and informing parties.
* Collaboration with designed pharmacy to ensure clients prescribed medications are obtained per the physicians
* Compile waiting list and provide ongoing contact/support to clients on the list.
*Key Skills and Abilities:*
* Strong communication, interpersonal skills, observation, listening, organization and time management skills.
* Strong skillset for group facilitation.
* Knowledge of and sensitivity to issues of gender, race, sexual orientation and issues impacting diverse communities.
* Possess a culturally sensitive lens when working with clients and facilitating family and client meetings
* A clear understanding of and ability to demonstrate professional ethics, boundaries, and judgement.
* Able to interact and communicate with others in an empathic and supportive manner while building effective therapeutic relations and rapport.

*The successful candidate will possess the following minimum qualifications:*
Caritas School of Life ? updated June 2021
* A minimum Bachelor?s degree and/or diploma in one of the sciences or a health-related discipline, with a considerable knowledge of substance abuse issues
* Significant knowledge and experience in varieties of counseling models including CBT, DBT and
Motivational Interviewing.
* Ability to facilitate psychoeducational and process groups to diverse clients with substance use issues
* Knowledge and experience in harm reduction strategies
* Excellent organizational and administrative skills
* The ability to work independently
* A working knowledge of data management systems
* A minimum of two years related experience in direct service delivery in the mental health and addiction field
* Demonstrated experience supporting individuals with mental health and substance use issues from a recovery and harm reduction approach.
* In good standing with an accredited regulated professional association.
* Fluency in a second language (French), this is a strong asset and highly preferred.
* A successful Police Vulnerable Sector Check, as per agency policy and procedures

* 8 hour shifts
* Holidays
* Monday to Friday
* Day/Evening
*Licence/Certification:*
* G Class Driver's Licence and a personal vehicle with an appropriate insurance (required).
* First Aid CPR training.
* ASIST training an asset
* GAIN3 Certified
